Hair Loss: Causes, Types, and When to See a Dermatologist

Hair loss affects millions of people worldwide, both men and women. Whether you're noticing more hair in your shower drain, experiencing a receding hairline, or seeing bald patches, hair loss can be distressing and impact your confidence. However, understanding the type of hair loss you're experiencing and the available treatment options is the first step toward addressing the problem.

Normal Hair Loss vs. When to Worry

Before we dive into concerning types of hair loss, it's important to know that some hair shedding is completely normal. The average person loses 50 to 100 hairs per day, which may sound like a lot but is actually within the healthy range. Hair naturally goes through growth cycles, and hairs in the shedding phase fall out to make room for new growth.

The good news: androgenetic alopecia responds well to early intervention with medications like minoxidil and finasteride, especially when treatment begins before significant hair loss occurs.

Telogen Effluvium: Stress-Related Hair Loss

Telogen effluvium is a type of hair loss triggered by significant physical or emotional stress, illness, major surgery, extreme dietary changes, or postpartum hormonal shifts. During highly stressful periods, a large number of hair follicles enter the telogen (shedding) phase simultaneously, leading to sudden, noticeable hair loss.

Common triggers include:

  • Postpartum hormonal changes (one of the most common triggers in women)
  • High-stress life events (job loss, relationship changes, grief)
  • Severe illness, high fever, or acute infection
  • Major surgery or hospital stays
  • Rapid weight loss or extreme dieting
  • Thyroid disorders or hormonal imbalances
  • Nutritional deficiencies (particularly iron, zinc, and B vitamins)

The positive aspect of telogen effluvium is that it's often temporary and reversible. Once the stressor is removed or the underlying condition is treated, hair typically regrows. However, if the shedding persists beyond six months, professional evaluation is important to rule out other conditions.

Alopecia Areata: Autoimmune Hair Loss

Alopecia areata is an autoimmune condition in which the immune system mistakenly attacks hair follicles, leading to patchy hair loss. This condition typically develops suddenly and can affect not just scalp hair but also eyebrows, eyelashes, and body hair.

Alopecia areata often appears as circular or oval bald patches with clean edges and no obvious inflammation or scarring. The hair loss can progress from a few small patches to more extensive baldness (alopecia totalis affecting the entire scalp) in some cases.

While there's no permanent cure, several treatments can help manage alopecia areata and potentially regrow hair, including topical and intralesional corticosteroids, minoxidil, and newer biologic medications. Early intervention by a dermatologist is crucial for the best outcomes.

Traction Alopecia: Hair Loss from Tension

Traction alopecia results from chronic tension on hair follicles caused by tight hairstyles. This type of hair loss is preventable and reversible if caught early, but can become permanent if the tension persists for years.

Common culprits include:

  • Tight braids, cornrows, or dreadlocks
  • Extremely tight ponytails or buns
  • Hair extensions applied with excessive tension
  • Frequent use of tight hair accessories

The solution is straightforward: release the tension by changing your hairstyle. Wearing your hair down, switching to loose styles, or avoiding pulling hairstyles allows damaged follicles to recover. If caught early, hair will typically regrow within weeks to months after eliminating the tension.

Available Treatment Options

Minoxidil (Rogaine) is an over-the-counter topical treatment available in foam or liquid form. It helps extend the growth phase of hair and can slow hair loss and potentially regrow hair. Results typically take 3-6 months to become visible.

Finasteride (Propecia) is an oral prescription medication for men that blocks the conversion of testosterone to DHT. It's FDA-approved and can stop hair loss progression and regrow hair, particularly for crown thinning. Women cannot use finasteride as it may cause birth defects.

Prescription and in-office treatments depend on the diagnosis. Alopecia areata often responds to corticosteroids injected directly into the affected patches. Inflammatory and scarring types of hair loss need the underlying inflammation treated before regrowth is possible. Identifying the right category is the whole point of the evaluation.

Lifestyle modifications including stress management, adequate sleep, proper nutrition, and managing underlying conditions like thyroid disorders or nutritional deficiencies can support hair health and reduce hair loss.
