Before Your Appointment: How to Prepare

Preparation for a full-body skin exam is simple and straightforward. A few days before your appointment, remove any nail polish so the dermatologist can examine the nails and the skin underneath. On the day of your appointment, wear your hair down or in a way that doesn't cover your scalp, as the dermatologist will examine your entire scalp for any suspicious lesions or abnormalities.

Before your exam, make a mental note of any spots or moles you're concerned about. During your consultation, the dermatologist will ask you to point out any areas that have changed, grown, or started itching or bleeding. It's also helpful to bring a list of any skin concerns or changes you've noticed over the past year, as this helps the doctor prioritize which areas to examine more closely.

During the Exam: What Happens Step by Step

Changing into the exam gown: You'll be provided with a private room to change into an exam gown. You may keep your underwear on if you prefer, though the dermatologist may need to examine the skin beneath if you have concerns in those areas. The gown allows easy access to all areas of your body while maintaining dignity and privacy.

Systematic head-to-toe examination: The dermatologist will conduct a thorough, systematic examination of your entire body. This typically begins with your scalp, examining the hair-bearing areas carefully for any abnormalities. They'll then move to your face, ears, neck, and work downward through your trunk and extremities. The examination includes often-forgotten areas like between your toes, under your nails, your genital area, and the backs of your legs; these are places where melanoma and other skin cancers often develop and go unnoticed.

Dermoscopy: For any concerning moles or skin lesions, the dermatologist will use a specialized magnifying instrument called a dermoscope. This handheld device allows high-magnification viewing of the skin structure and pigmentation patterns, making it easier to identify characteristics of skin cancer or benign lesions. Dermoscopy is non-invasive and painless.

Duration: A complete full-body skin exam typically takes 10-15 minutes. The exact time depends on how many moles or lesions you have and whether the dermatologist needs to examine any areas more closely.

After the Exam: Discussion and Next Steps

Once the physical examination is complete, the dermatologist will discuss their findings with you. They'll explain what they found, whether any lesions are concerning, and what your next steps should be.

Reassuring findings: If all skin lesions appear benign and healthy, you'll simply be advised to continue sun protection practices and return for annual monitoring. Most people with good skin health and no concerning lesions should have annual exams.

Monitoring: If you have multiple moles or a history of skin cancer, the dermatologist may recommend more frequent exams (every 6 months) and may take baseline photos of your moles for comparison at future visits.

Biopsy: If a lesion appears suspicious, the dermatologist may recommend a biopsy. A small tissue sample is removed and sent to a pathologist for microscopic examination. This is the only way to definitively diagnose skin cancer. Biopsies are quick, can be done in-office, and typically involve minimal discomfort.

Removal: If a lesion is benign but bothersome (perhaps for cosmetic reasons or because it's frequently irritated), the dermatologist can remove it during the same visit if you choose.

Do I need to be completely undressed? No. You'll wear an exam gown that provides coverage while allowing access to necessary areas. You can keep your underwear on, and the dermatologist will work around your clothing as needed.

Is the exam painful? Not at all. A full-body skin exam is completely painless. The dermatologist is looking and sometimes lightly palpating (feeling) the skin, but there are no needles or instruments involved unless a biopsy is recommended and you choose to proceed.
